An A5 or Dokibook-sized notepad. If you have a Filofax A5/Kikki-k/Dokibook or similar planner, you could easily punch holes in the sheets and integrate them into your planner. The notepad consists of 25 sheets of 100 gsm paper and a cardboard backing sheet.
